Two-dimensional phlebology techniquesTwo-dimensional techniques, sclerotherapy, phlebectomy and saphenectomy attack the visible varicose veins, which represent the effect of the pathology, not its cause. Sclerotherapy, phlebectomy and saphenectomy cannot be regarded as “curative” of venous insufficiency. They are techniques that destroy small portions of the superficial circulation, which are merely the effect of the pathology. The visible vessels constitute the “escape valve” for the insufficiency of the underlying perforating veins. If these vessels of the superficial circulation are obliterated and removed, the untreated underlying hypertension will continue to break through to the surface, giving rise to other varicose veins and telangiectasias. The same thingh appens if only a part of the miopragic circulation is treated. In this case, the varicose disorder will continue to evolve and the patient will be obliged to undergo further treatment, without obtaining the hoped-for benefit. |
